Upper trapezius spasm
An upper trapezius spasm is a sudden, involuntary tightening of the upper portion of the trapezius muscle—the broad muscle that runs from the base of the skull down the neck and across the shoulders. When it “grabs,” it can create intense tightness or cramping and noticeably limit neck and shoulder movement.
Common symptoms include:
Localized pain/tightness at the base of the neck, top of the shoulder, or between the shoulder blades
A sharp, stabbing, or dull cramping sensation (often with a palpable “knot”)
Reduced mobility: difficulty turning the head, tilting the neck, or lifting the arm comfortably
Tension headaches: trigger points can refer pain into the back of the head
Upper traps are highly reactive to posture and stress. Common triggers include:
Poor posture / “tech neck” (laptops, phones, prolonged sitting)
Stress and tension (unconsciously shrugging shoulders or clenching)
Overuse/repetitive load (heavy bags on one shoulder, repetitive overhead work)
Muscle imbalance (weak deep neck flexors/lower traps causing upper traps to overwork)
Many spasms settle within a few days with conservative care:
Heat to relax muscle fibers
Gentle stretching (ear-to-shoulder stretch, controlled chin tucks)
Soft tissue work (massage, foam roller, tennis ball trigger point release)
Ergonomics + movement breaks (monitor at eye level, frequent posture resets)
Light mobility and breathing work to reduce guarding
HyperWave EMF-Focused Shockwave Therapy is a non-invasive, drug-free option that can be especially effective for stubborn upper trap spasms and myofascial trigger points—when stretching and manual therapy haven’t provided lasting relief.
HyperWave may help by:
Reducing pain by desensitizing irritated nerve endings and calming spasm patterns
Supporting circulation and tissue recovery in tight, oxygen-starved muscle areas
Helping release trigger points/adhesions that keep the muscle locked “on”
Improving comfort so posture retraining and strengthening can actually stick
In-office treatment: gel + a handheld applicator over the neck/shoulder region
Technique: often combined with “pin and stretch” (pressure on knots + gentle active movement)
Time: typically 3–5 minutes on the upper trap area (pulse-based treatment)
Plan: commonly 3–4 sessions over a couple of weeks
